Specialist Support Coordination referrals for new and returning participants
Coordination Practice Australia (CPA) accepts referrals for Level 3 Specialist Support Coordination from participants, families, support coordinators, LACs, clinicians, and other professionals. We focus on complex cases where participants face multiple barriers, high risk, or involvement with multiple systems (NDIS, health, justice, housing, child safety, etc.).
When to refer to CPA
A referral to CPA is appropriate when an NDIS participant:
Has complex disability, health, or psychosocial needs that are not responding to standard support coordination
Is experiencing crisis, significant risk, or plan instability
Is navigating multiple agencies or systems (e.g. hospitals, mental health services, justice, child safety, housing, disability services)
Needs targeted, time-limited specialist input to resolve barriers, stabilise supports, and build capacity
Requires strong coordination across providers and clear, evidence-based recommendations for plan reviews
What referrers can expect
When you refer to CPA, you can expect:
Clear intake and triage – We review referrals promptly and confirm whether the case fits our specialist scope and current capacity.
Transparent communication – With participant consent, we keep referrers informed about key milestones, barriers, and outcomes.
Focused, time-limited intervention – Our work is designed to resolve specific complex issues and build participant capability, not to create long-term dependency.
Robust reporting – We provide concise, practical reports for plan reviews, including clear recommendations about supports, risks, and next steps.
How to make a referral
1. Check eligibility and fit
Before referring, please confirm:
The participant has an active NDIS plan with Specialist Support Coordination (Level 3) funding, or a clear rationale for requesting it at the next plan review
The case involves complexity or high risk that aligns with CPA's specialist focus.
The participant (or their nominee) consents to the referral and to information sharing as needed.
2. Complete the referral form
Please use CPA's Referral for Specialist Support Coordination form. The form captures:
Participant details (name, DOB, contact, NDIS number)
Plan details (start/end dates, management type, relevant budgets)
Referrer details and role
Brief summary of needs, risks, and current supports
Goals for specialist support coordination
Consent and information-sharing preferences
Optional attachments (current NDIS plan, relevant reports, risk or behaviour support plans)

3. Intake and response
We aim to acknowledge referrals within 1 business day
If the case is suitable and we have capacity, we will contact the participant (and referrer, with consent) to arrange an initial conversation.
If we are unable to accept the referral (due to scope, risk profile, or capacity), we will provide brief feedback and, where possible, suggest alternative options.
Information we find most useful
To help us triage referrals efficiently, please include (where available and with consent):
Current NDIS plan (or most recent plan summary)
Any risk, behaviour support, or safety plans
Recent clinical or allied health reports that describe functional impact and complexity
A short summary of key barriers, recent crises, or system issues
Details of current supports and providers, including any conflicts or breakdowns
Participant goals and priorities for the next 3–6 months
You do not need to provide all of these items for a referral to be considered, but the more relevant information you can share (with consent), the faster we can assess and respond.
