Yes. CPA works with self-managed plan-managed participants. Please indicate the plan management type on the referral form
CPA currently accepts referrals for participants in Brisbane and surrounding areas, with capacity for remote specialist coordination where appropriate
No. CPA specialises in Level 3 Specialist Support Coordination for complex cases. We do not provide ongoing general support coordination (some L1/L2 support coordination may be provided to support client transition and continuity of care)
With participant consent, we prepare concise reports and recommendations to support plan reviews, focusing on risks, barriers, outcomes achieved, and recommended supports going forward. We can share these with the participant, referrer, and NDIA as agreed
